The Legacy Society

By naming Phoenix Society as a beneficiary in your estate plan, you can become a member of our Legacy Society. Members of the Legacy Society are recognized in each issue of Burn Support Magazine and our annual report. They also receive other exclusive benefits.

To confirm your enrollment, please complete the Bequest Notification Form substantiating your confidential wishes. The agreement is non-binding and does not constitute a legal promise of any future donation to Phoenix Society. Please send the completed form to:
